Description

Plans, coordinates, and advocates with court personnel in an effective and efficient manner to identify and select appropriate referrals to the mental health and/or substance abuse outpatient program through interviews and assessment. Trained on gender specific, trauma informed, and culturally sensitive best practices and assessments. Provides robust individualized and gender-specific ongoing advocacy; monitors and supports each participant in the mental health and/or substance abuse outpatient program throughout the course of treatment. Advocates for gender-specific, therapeutically effective best practices, services, incentives, and sanctions. Establishes and maintains effective communication and interaction between judges, prosecutors, correctional staff and treatment staff. Responds to crises and provides immediate liaison for intervention to client working with the court system. Serves as a key participant in outreach in courts. Provides advocacy for clients at court appearances to assist with continuum of treatment based on clinical recommendations. Acts as the primary mediator between the criminal justice system, the treatment program and the Bridges the criminal justice system with the mental health and substance abuse systems and manages cross-system interactions. Provides tracking, outreach, and advocacy to post discharge court reviews to ensure client/graduate is receiving the best possible outcome/sentencing within criminal justice system. Establishes and maintains a compliance document and monitoring system to include: outreach logs, the number of client screenings, and the number of clients referred to each program. Provide immediate notification of non-compliant Prepares and provides written reports summarizing the participant's progress as requested by the Courts. Collaborates with judges, prosecutors, and substance abuse outpatient staff on each case. Attends interdisciplinary team meetings and case Assist in program development of any diversion program deemed necessary by the court.
