This is a Mental Health (MH) Case Management position based at Cameron Elk Behavioral and Developmental Program (CE) working with Mental Health individuals who are incarcerated in the Elk County Prison, developing their reentry plan as well as working with the homeless population. DESCRIPTION OF WORK

The Caseworker will develop a working knowledge of the adolescent and adult service system in order to facilitate effective collaboration between agencies, act as a resource, and make appropriate referrals for services. Other job duties include:

Attends meetings/trainings that will enhance knowledge of Housing and Services; collaborates with the CE Housing Specialist and other available resources. -
Works collaboratively with Social Service Agencies, Intermediate Units, School Systems, the CASSP System, adolescents and their families to gain referrals and assist in coordination of services. -
Monitors caseload for HMIS Tracking. -
Assists MH Program Specialist II in providing Case management Services to CE consumers receiving inpatient care at Warren State Hospital, participates in treatment team planning, and reviews and facilitates discharge planning and referrals for aftercare services. -
Assists MH Program Specialist II in working closely with provider agencies in facilitating the most appropriate diversion of MH consumers from unnecessary state hospitalizations and the least restrictive level of care. -
Ensures that services are consistent with the philosophy of the Recovery Model of community-based mental health care. -
Performs other related duties as expected.
Work Schedule and Additional Information:
- Full-time employment.
- Work hours are 8:00 AM to 3:30 PM, Monday - Friday, with a 30-minute lunch; OR 7:00 AM - 4:15 PM, Tuesday - Friday, with a 30-minute lunch.
- This position includes on call daytime MH delegate services on a rotating schedule during business hours.
- Travels throughout the MH/ID jurisdictions as required.
- You may have the opportunity to work from home (telework) part-time. Remote work option is subject to approval from supervisor as proficiency increases. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Ridgway, Pennsylvania.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months of experience as a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- A bachelor's degree with a social welfare major; or
- A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
- Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
Other Requirements:
- PA residency requirement is currently waived for this title.
- Hiring preference for this vacancy will be given to candidates who live within Elk or Cameron County. If no eligible candidates who live within Elk or Cameron County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
