Position Details General Responsibilities: * Maintain proper medical record documentation in accordance with Merakey, DPW, DOH regulations and policies. * Provide intake services by evaluating incoming referrals to determine the appropriate treatment intervention. * Prepare diagnostic assessments, case histories and other related correspondence as per Merakey, DPW and DOH policies and regulations. * Develop Treatment Plans: Develop a preliminary treatment plan based on client's presenting problem. The preliminary treatment plan is to be completed by the end of the second counseling session. A Master Treatment is to be developed by the end of the fourth counseling session. The therapists and client will identify individualized treatment issues, long and short term goals, and the type and frequency of services to meet those goals. Therapists will review and revise treatment plans with the client every 180 days. The Master Treatment plan and subsequent Treatment plan updates are to be reviewed and signed by the client, therapists, clinical supervisor and psychiatrist or supervisory physician. * Provide ongoing therapy and supportive counseling services to assigned caseload. * Monitor client progress in treatment and recommend treatment plan revisions and/or other appropriate disposition. * Maintain accurate and professional case records, documenting each case activity. Complete paperwork in a timely fashion. This includes, but is not limited to, client vouchers, progress notes, treatment plans, aftercare plans, case consults, discharge summaries, assessments, letters and other client related correspondence. * Attend and participate in meetings as assigned. * Accept responsibility in solving problems and improving delivery of services to clients and the community. Accept and perform related work as required by immediate supervisor and the Program Supervisor. * Attend regularly scheduled supervisory conferences with immediate supervisor, prepared to review cases, treatment plans, and other diagnostic materials. * Develop an individualized training plan with supervisor on an annual basis. * Adheres to all Merakey, County, State, and Federal policies and regulations related to mental health treatment services. * Complies with Merakey ethics and codes of conduct, as well as the ethics and code of conduct of own discipline, in all contact with clients, co-workers, and the community. * Participates in Merakey quality improvement/quality assurance activities. * Maintain confidentiality regarding client and department information as per policies and regulations of Merakey, Department of Public Welfare (DPW) and PA Department of Health (DOH). * Other duties as assigned to meet programmatic needs.
