Location: Central City Concern - Blackburn Center

Schedule: Monday - Friday (12:30pm - 9:00pm)

Compensation:



  • SUDS Counselor R (CADC-R): $23.36 - $30.37/hour
  • SUDS Counselor I: $25.76 - $33.49
  • SUDS Counselor II: $28.40 - $36.92


MINIMUM QUALIFICATIONS (REQUIREMENTS)

*In order to be granted formal consideration for the SUDS Counselor I position, please ensure your professional/working experience reflects the minimum requirements below:



  • Required: Must have High School Diploma or GED. Advanced degree preferred.

SUMMARY:

The Blackburn Center is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations. This position will work as a member of a multi-disciplinary team at the Blackburn Center to deliver comprehensive, person-centered, integrated services to the Portland Eastside community. The SUD Counselor works as a member of a treatment team providing substance abuse treatment and case management. The team works to address all treatment needs, including crisis management, substance abuse treatment, physical health management, mental health management, DUI, accessing affordable housing, accessing employment and educational opportunities.

The SUDS Counselor R works as a member of a treatment team providing substance abuse treatment and case management under direct supervision.

The SUDS Counselor I works as a member of a treatment team providing substance abuse treatment and case management under general supervision. Performs work undergeneral supervision and the incumbent is expected to become able to independently perform the full range of duties.

The SUDS Counselor II works as a member of a treatment team providing substance abuse treatment, case management, and may provide functional supervision to lower levels.

RESPONSIBILITIES:



  • Conduct screening and assessment of potential clients to determine treatment needs, placement, and diagnosis; develop an individualized treatment plan to reflect these
  • Participate in direct treatment of clients by:

    1. Individual counseling
    2. Group counseling as needed
    3. Provide case management and care coordination


  • Adhere to agency norms for completion of all clinical documentation, including but not limited to assessment, treatment plan, progress notes, monthly reports to referral sources, and service conclusion summaries.
  • Participate in all Quality Assurance activities.
  • Participate in clinical meetings.
  • May facilitate outpatient treatment groups for driving under the influence (DUI).
  • Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
  • Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  • Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
  • Attend all mandatory CCC trainings in a timely manner.
  • Document client services in electronic health records within 72 hours.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Perform other duties as assigned.

  • Supervise practicum students when needed.
