Do you have a passion for our community and the skill set to support growth at an up-and-coming health organization? Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers. Does not require working late nights, no holidays, and no weekends! Who are we?
Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services that include family medicine, internal medicine, women's health, urgent care, dental, radiology and integrated behavioral health. We also offer supportive services such as case management, health education, and benefits enrollment.
What will you do?
The Behavioral Health Navigator involves implementing care plans designed by social workers to address patients' identified needs, resources, and goals. Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources. The position promotes patient self-management and independence, assisting with essential services like transportation and housing. Additionally, the role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards. The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential. Schedule: Monday - Friday, 8 a.m. - 5 p.m. Responsibilities

It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to their orientation.
