Wage Range: $75,000 - $85,000, depending on experience.

Schedule: Monday - Friday 8:00am-5:00pm

Location: Providence Place (Minneapolis), Woodbury Senior Living (Woodbury) and Birchwood Senior Living (Forest Lake)

Responsible for supporting the operations of the Social Services department across the facilities in their assigned region. This position is intended to educate, coordinate, and consult in providing for each resident's social, emotional, and psychological needs and the continuing development of the resident's full potential from admission to discharge. Additionally, this position is accountable for supporting the Full Circle/Death Doula program and cultivating a strong relationship with hospice & home care services across the facilities in their assigned region.

Position Functions and Responsibilities



  • Support the social services department in the assigned region
  • Provide education, leadership, and consultation to social workers in skilled nursing settings.
  • Facilitates and conducts personal marketing visits with the purpose of networking with key referral sources, such as discharge planners, community leaders, and county case workers.
  • In the absence of a Director of Social Services, performs specific duties outlined in the respective job description in an interim capacity
  • Assist, as needed, with coordination of family groups to promote communications, education and support between family members, facility staff, and administration and provide counseling
  • Assist in the education of the community regarding aging, resident rights, facility services and other related topics.
  • Coordinate with the social services departments any admission/discharge challenges, including risk management reviews.
  • Participate regularly with Interdisciplinary meetings and other community meetings including Care Plan meetings and Quality Improvement.
  • Ensure respective Minimum Data Set assessments are completed proficiently and timely.


Qualifications



  • Valid state licensure as a Social Worker.
  • Post Secondary Education with a Bachelor's degree in Social Work or human services field including but not limited to Sociology, Psychology, Special Education, or Rehabilitation Counseling.
  • A minimum of five years in long-term care or acute care is preferred.
  • Previous supervisory experience in a health care setting directly overseeing individuals.
  • Three years of supervised social work experience in a health care setting working directly with individuals.
  • Must be able to relate professionally, positively, and cooperatively with residents, resident's families, and employees.
  • Must possess proficient computer skills, including email use.
