The MDS Coordinator is responsible for coordinating the care delivery interventions for residents in their assigned area in accordance with current professional practice standards, physician's orders, facility policies and procedures, and local, state and federal regulations and within the scope of practice.
Essential Functions Statement(s)
- Ensures all care is provided with respect to privacy and confidentiality
- Follows all established organizational and OSHA policies, rules and regulations concerning safety, sanitation and infection control
- Attends and participates in scheduled training, educational classes and meetings to maintain current certification as applicable
- Attends and participates in in-service training programs as mandated by regulatory agencies and facility policy
- Participates in Performance Improvement program
- Participates in assessing, planning, implementing and evaluating resident care delivery with actual outcome
- Assess, plans, implements, and evaluates services and education to be delivered to the family or significant other in coping with and participating in the management of the resident's care
- Assists in completing the initial and ongoing nursing assessments needed to plan care and interventions
- Makes resident rounds to ensure care delivery meets quality standards and restorative protocols
- Utilizes appropriate infection control techniques to prevent spread of infection
- Administers prescribed medications and treatments as required, in accordance with approved nursing protocols
- Observes vital signs and hydration status to detect deviations from normal limits
- Assembles and utilizes resident care equipment and ancillary supplies
- Assists in receiving, transcribing and implementing physician's orders
- Records resident's responses to treatments/medications, response or lack of progress toward care plan or restorative goal
- Assists in documenting and observing changes in condition and unusual incidents and reports findings to supervisor, physician and responsible party, as required
- Administers proper assessment skills and techniques as appropriate, including the following: Gastrointestinal, genitourinary, pulmonary, cardiac, circulatory, neurological, musculoskeletal, and integumentary assessments; Positioning techniques; Transfer techniques; ROM exercises; Emergency procedures: properly administers oxygen, suctions, performs CPR, and uses the ambu bag; Performs glucometer checks
- Administers IVs through the following: Conducts IV site care; Mixes, hangs and administers IV solutions to patients; Computes rate of IV fluid administration; Operates the infusion pump
- Performs suctioning, as necessary including tracheotomy, nasotracheal and Tracheotomy care
- Administers gastrostomy care through proper irrigation and placement check
- Applies proper sterile and isolation techniques as necessary
- Collects lab specimens as required
- Complies with the State regulations specific to RNs and LPNs in the insertion, administration and care of IVs; also demonstrates knowledge of and monitors for potential complications related to central lines and PICC lines
- Consults with dietary regarding the special dietary needs of the residents and follows through on dietary recommendations
- Assists in monitoring residents' progress, lack of progress, or changes in condition, which is described in the medical record
- Monitors residents with significant weight loss and gain and identifies those at risk for nutritional problems and implements preventative measures reports as well as reports observations to the physician and dietary
- Completes all forms used throughout the residents chart according to policy and procedures
- Gives and receives report of significant changes in resident condition
- Completes all documentation responsibilities, including those for new admits; general condition; ill resident reporting; 24-, 48-, or 72-hour follow up on infections; medications; treatments; prn's; 24-hour reports; intake and output reporting; and the monthly medication renewal process; new/changed diet orders and forwards information to the Dietary Department as necessary; any discrepancies noted concerning physician's orders, diet changes, charting errors, etc, and reports to supervisor as needed
- Assists in maintaining unit records
- Completes the care plans (acute and long term) and is involved in the care conferences
- Interpret policies/procedures to personnel, residents, visitors, and government agencies as required
- Ensures residents charge slips are completed and forwarded to the appropriate department for processing
- Performs administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc, as necessary
- Completes the MDS and is knowledgeable in the MDS, Medicare and PPS process
- Evaluates the effectiveness of the care interventions, identifies problems and trends and develops alternative interventions
- Participates and conducts (as appropriate) investigations related to any alleged abuse
- Has knowledge and understanding of Resident Rights
- Ensures progress notes are reflective of the care plan and approaches on the care plan are being followed
- Ensures assigned nursing assistants are aware of the resident care plans, and ensures nursing assistant refers to the care plan prior to administering care
- Understands and executes the procedures for all admission, discharge and transfer processes
- Coordinates and evaluates the supportive interventions provided by nursing to other departments or services to achieve a holistic approach to resident care
- Coordinates and evaluates the efforts of other departments/services in providing education and support to family members in coping with and participating in the management of the residents' care needs
- Assists physicians, when necessary, during rounds and examinations
- Participates in the assessment, planning and implementation of nursing staff in providing resident care
- Participates in planning and implementing the coordination and involvement of nursing staff with other departments/services to teach family the management of the resident's care
- Monitors performance of nursing personnel to establish a high standard of care delivery and accurate documentation, which reflects resident needs, progress, and condition
- Assists with sending requisitions to initiate physician orders
- Assists in planning and conducting training for other nursing personnel
- Assists in initiating and coordinating appropriate communication concerning resident to and from family and significant others
- Communicates with interdisciplinary team, evaluate findings, and proposes adjustments to plan of care
- Observes resident including abnormal lab values, response to treatments/meds, and significant incidents, and notifies nursing supervisor, physician, and when indicated, the family of change
- Coordinates telephone inquiries, when necessary
- Assists in reviewing resident care plans with resident, supervisor, resident's physician, certified nursing assistants, and when appropriate, the resident's family
- Adheres to current practice standards in assigned area and maintains records by documenting findings on a consistent basis
- Identifies and maintains accountability for security within assigned areas of responsibility, ensuring employees understand their obligations to protect company and employee property and takes appropriate action when security or safety infractions occur
- Maintains confidentiality of necessary information
- Coordinates work of department with work of other departments
- Attends and participates in staff, departmental and other community meetings and sits on required committees
- Assists in developing and updating departmental procedures and policies
- Maintains required records and reports as outlined in UCH's Policies and Procedures manuals
- Participates in department budget planning and generates monthly report, as requested
- Handles unusual occurrences calmly and logically to maintain continuity of business and duties
- Maintains appropriate, effective communication with residents, and with residents' family or other significant relationships, and ensures departmental staff does the same
- Follows all appropriate safety and security guidelines, care procedures and protocol for residents in the Dementia Care Unit
- Participates in relevant educational and training activities as appropriate
- Performs all other duties as assigned or directed

Skills & Abilities
- Education: Associate's Degree (two year college or technical school) Preferred, Field of Study: Nursing - Successful completion of a state-approved pre-licensure nursing education program: Required
- Experience: Six (6) months or more of related experience
- Computer Skills: Must be able to proficiently operate a computer, the Internet and basic office equipment
- Certifications & Licenses: Passing of National Council Licensure Examination-Registered Nurse (NCLEX-RN) or NCLEX-Practical Nurse (NCLEX-PN); Active RN or LPN license conferred by the Ohio Board of Nursing; Must participate in Continuing Education; Must obtain and maintain mandatory, state and federal requirements and certifications for practice or occupation; Must be CPR certified
- Other Requirements: Previous experience in long-term care, preferred; Clinical experience preferred, prior MDS experience preferred but not required; Must be able to read, write, understand and speak the English language; Must be able to work with elderly residents, families and other health care staff members; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
