Responsibilities
  • Provide personalized, one-on-one physical therapy care to patients in their homes, focusing on helping them regain independence and improve their quality of life.
  • Collaborate with patients, families, and the care team to develop and implement individualized care plans that align with patient goals.
  • Participate in a nationally recognized healthcare team by ensuring high-quality physical therapy care and contributing to a supportive, patient-focused environment.

Qualifications
  • Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.
  • Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.
  • Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation
  • One year experience caring for similar patient population as to be assigned.
