Title: LTSS Service Care Manager

Location: member facing (Source from Corpus Christi)

Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):

LVN needed to assist with follow up calls for members.
LVN will work assigned tasks
LVN may perform field visits in Corpus Christi area if needed

LVN will fill put required forms for each assessment
