Overview

Private Duty LPN - G-Tube Care (Hornbeak, TN)

Overview:
Join our caring team as a Private Duty LPN, providing in-home care split between Hornbeak, TN, and Jackson, TN. You'll work the first half of the month in Jackson and the second half in Hornbeak, enjoying variety while making a meaningful difference.

Location & Schedule:

Hornbeak, TN: 15th through the 30th of each month

Week 1: Monday, Tuesday, Thursday, Friday (7:30am-5:30pm)
Week 2: Monday, Tuesday, Saturday, Sunday (7:30am-5:30pm)

Jackson, TN: 1st through the 15th of each month
- Same schedule split applies

Qualifications:

Current LPN license
G-Tube care experience required
CPR certification

Responsibilities

Perform selected nursing procedures within the scope of LPN practice
Provide patient and family teaching according to plan outlined by the RN
Monitor patient's vital signs and make specific measurements and observations
Monitor and record intake and output. Report significant changes immediately
Observe, assess and record patient's physical and mental condition and reports any changes
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

Licensed Practical Nurse / LPN license in good standing
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Active CPR certification
Familiarity with electronic health records
Strong problem solving and critical thinking skills
Reliable transportation, valid driver's license and current automobile insurance
