Carle Health is seeking an additional advanced practice provider to join our team in Champaign-Urbana, Illinois.


Opportunity Details

  • Work with a collaborative team including the Geriatricians, Geriatric APPs, nurses, and nursing home staff to provide patient care to the full spectrum of senior care which includes nursing home, assisted living and memory care patients in the Champaign-Urbana area.
  • Typical schedule M-F / 8:00a-5:00pm with rotating paid weekend and weekday overnight call (do we want to include paid here?) I think yes

Essential job functions:

  • Typically cares for patients in at skilled nursing facilities and may include assisted living facilities and memory care centers
  • Works in collaboration with attending physician.
  • Works closely with extended care facility staff to coordinate patient care.
  • Provides health assessment for patients directed toward continuing a state of wellness, establishing the need for intervention, evaluating responses to and outcomes of therapy, and following a mutually agreed upon treatment plan.
  • Orders labs & diagnostic testing, follows up on results.
  • Ensures documentation of care is complete and accurate. Charges for services rendered.
  • Communicates with other health professionals, families, and referral agencies regarding health status and needs of particular patients or patient groups. Refer to specialists as needed.
  • Serves as a role model in the enhancement of nursing practice and professional conduct following professional practice guidelines.
  • Communicates with appropriate medical personnel regarding patient condition, history, treatments, response treatments and other relevant information.
  • Acts as preceptor and/or participates in orientation of new employees.
  • Rotates night/weekend call with Carle's SNFist team
  • Participates in quality improvement projects

Education Requirements:
Master's degree in Nursing or Physician Assistant
Background working with Geriatric patients is ideal, but team is willing to train New Grads

Licensure/Certification Requirements:
Current Illinois registered nurse license and advanced practice nurse or physician assistant

The compensation range for this position is $55.13 - $71.67/hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
