Sabathani Community Center (SCC) is a 58-year-old African American institution and nonprofit serving a multi-cultural community of 50,000 community members in South Minneapolis. SCC was founded in 1966 by a handful of concerned community members seeking to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education. Wages & Benefits

$25 - $30 per hour; this position is not eligible for benefits except for participation in the agency sponsored retirement savings plan (with a 4% employer match and immediate vesting) and accrual of Minnesota Sick and Safe Time.

Schedule

This is a temporary, full-time (40 hours/week), and hybrid position (projected to end in May 2025) that includes some evening work on Tuesdays and Thursdays, and some weekend work.

A Typical Day in the Life...

A typical day as Sabathani's Family Coordinator includes the opportunity to provide supervised visitation services to families who are involved with Child Protection to support families whose children are in out-of-home placement or are at risk for placement. Other responsibilities will include the following:



  • Scheduling and providing transportation for clients as outlined in the case plans (including being available for flexible scheduling 7 days per week).
  • Participating in case conferences and court appearances upon request.
  • Providing ongoing assessment and evaluation of clients' goals.
  • Monitoring and providing a safe visitation environment for children.
  • Assessing participants' readiness for case management services based on needs, ability, and willingness to participate in the program.
  • Creating case plans for participants that include short, medium, and long-term goals and the steps to achieve those goals.
  • Assisting in developing supportive parenting plans.
  • Establishing and maintaining collaborative working relationships with community resources.
  • Providing relevant referrals to agencies and service providers based on clients' goals and objectives.
  • Advocating for participants with agencies and service providers to minimize barriers to participants receiving needed services.
  • Conducting needs assessments to identify service gaps.



Who You Are



  • You have a Bachelor's Degree in Human Services/Social Services, or you have demonstrably equivalent knowledge gained through alternative courses of study and life experience.
  • You have a minimum of 2 years of work experience in Human Services/Social Services.
  • You have experience in non-profit management, case management, and/or program development.
  • You have experience working in the African American community (preferred).
  • You have strong written and verbal communication skills.
  • You demonstrate adaptability in the midst of difficult circumstances and emergencies.
  • You have a working knowledge of Outlook, Word, and Excel; PowerPoint and Access are a plus.
  • You are highly analytical with the ability to evaluate the impact of case management recommendations.
  • You are skilled in time management with the ability to set work priorities create solutions to work-related problems.
  • You maintain current CPR, AED, and BBP certifications (for new hires, training will be provided during onboarding.)
  • You have access to your own car for transporting clients.
  • You have a valid driver's license and the ability to meet Sabathani's driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
