Job Details Community Health Assistant Job Description PRIMARY FUNCTION: The Community Health Assistant will learn about the roles and responsibilities of a Community Health Worker (CHW), attend CHW training, shadow CHWs, and provide CHW services to community members/patients and their families under an experienced worksite supervisor and the mentorship of a CHW Mentor/Coach, who will provide professional development. Throughout the trainee period, the Community Health Assistant will gain more independence with the goal of being promoted to a CHW.
