Job Description

PRIMARY FUNCTION:

The Community Health Assistant will learn about the roles and responsibilities of a Community Health Worker (CHW), attend CHW training, shadow CHWs, and provide CHW services to community members/patients and their families under an experienced worksite supervisor and the mentorship of a CHW Mentor/Coach, who will provide professional development. Throughout the trainee period, the Community Health Assistant will gain more independence with the goal of being promoted to a CHW.

ESSENTIAL FUNCTIONS:

  • Attends and participates in all trainings

  • Completes all homework assignments and projects

  • Attends and participates in all coaching and mentoring sessions

  • Obtain an understanding of the roles and responsibilities of a CHW

  • Works collaboratively and effectively within a team

  • Builds and maintains positive working relationships with the community members, community leaders, providers, care coordinators, social workers, office staff, and others

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson

  • Understands the importance of practicing cultural humility when interacting with community members and others

  • Demonstrates clear communication and positive interpersonal skills
