TRC Case Manager Supervisor- FT Days (9am-5pm)- Trauma Program

The TRC Case Manager Supervisor offers comprehensive services to survivors of violent crimes, with a particular focus on
gunshot wound survivors. Responsibilities include conducting group and individual training sessions on topics such as social
and coping skills, as well as self-management techniques. The supervisor also provides service coordination and facilitates
connections to community resources tailored to each patient's needs, while actively engaging in crisis prevention and
management when necessary.
Responsibilities



  • Provides wrap around case management services.
    - Provides supervision to interns pursuing their Bachelor's degrees in a mental health related field.
    - Participates in multidisciplinary team.
    - Documents services according to TRC guidelines in established electronic medical record.
    - Establishes and facilities new program partnerships.
    - Provides service coordination and community linkage to services.
    - Engages in crisis prevention and management, when applicable.
    - Provides group and or/individual training based on patient needs and program objectives.
    - Engages in direct patient care with patients who are survivors of violent crimes.
    - Participates in community engagements on behalf of the TRC Program.
    - Accompanies patients to appointments and assists with transportation assistance.
    - Provides assistance to TRC Director and Data Analyst with data needs.
    - Assists TRC Clinicians with patient engagement/enrollment, as needed.
    - Performs other administrative duties or project tasks as assigned.
    Requirements
    Bachelor's Degree in Behavioral Science including Criminal Justice, Cultural Studies, Anthropology, or Sociology.
    In lieu of degree in Behavioral Science, will accept bachelor's degree in non-related field, along with six (6) years of
    comparable job experience with one (1) year of experience with the SPMI and/or Addictive Disease population, preferably in
    a full-time status.
  • A minimum of three (3) years of experience, with one (1) year of experience with SPMI and/or Addictive Disease population,
    preferably in a full-time status
  • BLS - American Heart Association
