Requisition ID req32755 Working Title Community Health Worker Position Grade 09 Position Summary We are seeking a compassionate and dedicated Community Health Worker (CHW) or Certified Peer Support Worker (CPSW) who is interested in making a change in the community and joining our team. The selected candidate will provide support services to clients using Trauma-Informed Care, Harm Reduction, and Client-Centered Approaches. This role involves working closely with clients to enhance their well-being by offering education and coordination of care aimed at improving social determinants of health and will play a crucial role in fostering positive health outcomes within the community. The CHW will work to support these individuals by addressing their unique needs and providing resources and guidance to improve their overall quality of life. Minimum Qualifications High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Preferred Qualifications *High school diploma or equivalent; Certified Community Health Worker (or be willing to obtain certification within 6 months) Bachelors in Social Work or 4 years of direct work with individuals who have experiences with the criminal justice system, homelessness, Co-Occurring and or substance use diagnosis. Any equivalent combination related to education and/or experience may be considered for the above requirements. *Knowledge of trauma-informed care, harm reduction, and client-centered care principles, community collaboration and resource navigation. *Ability to work independently as well as part of a multidisciplinary team. *Valid driver's license and reliable transportation, as home visits may be required. *Bilingual skills are a plus, depending on community demographics. * Ability to prioritize tasks, set deadlines, and maintain a clear schedule to balance multiple client needs and internal responsibilities. * Proficiency in using digital tools like email, Microsoft calendars , Zoom * Clear and concise communication, especially in remote settings, is crucial for keeping clients informed about timelines, deliverables, and any changes in schedules. * Ability to handle time-sensitive situations, such as meeting deadlines or resolving issues that could impact the schedule. * Flexibility to adjust plans and timelines based on client feedback or unforeseen circumstances.
