About the Position
As our Registered Nurse you will provide in-home and virtual nursing visits and case management services to high-risk infants and children and their families who have been in the nursery intensive care unit (NICU) so that the physical, developmental, social and environmental status of these babies is improved.

We Require
Bilingual skills in English/Spanish One year in pediatrics, NICU, and/or maternal/child health Bachelor's degree in Nursing with a valid unrestricted license as a Registered Nurse issued by the Arizona State Board of Nursing

Job Contributions
Completes family visits and related documentation for caseload, including High Risk Perinatal (HRPP) and other at-risk children (CYSHCN). These are done both at home and virtually Performs infant physical and developmental assessments and family knowledge and social assessment Provides appropriate family-centered education and interventions to maximize optimal growth and development of these at-risk infants and toddlers Performs case management of assigned families, including scheduling home visits at appropriate intervals, making referrals and advocating for community services Completes all required paperwork, computer input and production of individual report ina timely manner Participates in required quality assurance activities Attends assigned hospital discharge planning meetings, ADHS-sponsored HRPP meetings and required OFH and MCDPH meetings and trainings Attends all required educational conferences and participates in continuing education as appropriate to update knowledge levels Participates in community networking and advocacy. Provides community presentations related to infant and toddler issues and education as requested Participates in Newborn Screening Program, as needed, including home visits, telephone contacts, case management, education and training.
Working Conditions
Ability to exert 20-25 lbs. occasionally, 10-25 lbs. frequently, or up to 10 lbs. constantly May be subjected to moving mechanical parts, vibrations, fumes, odors, dusts, poor, ventilation, chemicals, oils, extreme temperatures, inadequate lighting, workspace restrictions, intense noises, and travel Valid Arizona Driver's license and insurance is required upon hire CPR BLS Certification Must secure and maintain an Arizona Department of Public Safety Level 1 Fingerprint Clearance Card and complete a background check Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test
