Title: Social Worker ( LSW/MSM Must ) Pay Rate: $44.64/hr. on w2 without benefits Duration: Till 06/30/2025 (possibility to extension) Location: Preakness Healthcare 305 Oldham Road Wayne, NJ 07470 Work Hours: 9am-5pm or 10am-6pm; lunch period: 1/2 hour Unpaid BG Check Required: Yes Dress Code: Business casual, except on Fridays Job Description: Perform resident assessments. Provide resident-centered care as a member of the interdisciplinary care team. Plan for resident discharges and maintain requires documentation. Provide counseling and guidance to residents and their families. Provides information on outreach services offered to families. Ability to establish and maintain cooperative relationships with clients and staff. Long-term care experience Social Worker in a nursing home facility. Must be able to assess/document in MDS. Mandatory skills/certification(s) Requirement: LSW, MSW.
