The Clinical Manager-Home Care - Community Division is responsible for the overall management and coordination of operational and clinical services for SJ patients receiving home care services in the assigned Division. This position oversees and leads staff providing care in homes. This position is estimated to spend 50% of the work week in the field with the team and the other 50% will encompass office-based administrative and operational responsibilities. In this role, you will:

  • Plan and oversee the work of professional and support personnel and contribute to the implementation of policies and procedures for the assigned division.
  • Be responsible for quality of nursing care, effective utilization of staff, and compliance with hospital policies.
  • Oversee the referral management and admission processes, and schedule staff and patients.
  • Serve as a liaison between the agency and the SJCRH hospital staff (i.e. APPs, MDs, Clinical Operations, Pharmacy, Rehab services, etc.) for patient related needs.
  • Be responsible for the development of staff to meet organizational and professional standards, for assigned division, that support the mission and vision of the Agency and SJCRH.

This role has 24-hour accountability for the assigned division and serves as the vital link between the administrative strategic plan and the point of care. Supports the Agency as a change agent, oversees the throughput of the assigned Division, ensures competency and professional develop of staff to meet organizational and professional nursing standards.

Day shift with on call requirements (weekends, holidays, etc.)

Minimum Education and/or Training:

  • Bachelor's degree in Nursing (BSN) required
  • MSN strongly preferred

Minimum Experience:

  • Five (5) years of clinical pediatric experience in an acute or ambulatory setting required.
  • Two (2) years of Home health, palliative care or hospice experience preferred.
  • Two (2) years of supervisory/management experience required.

Licensure, Registration and/or Certification Required by Law:

  • (RN-TN) Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state
  • (RN-AR, RN-MS )Must possess a current State Board of Nursing license in the state of primary residence if the state is a Nurse Licensure Compact state
  • (DLD- RESIDE) If operating a motor vehicle for St. Jude business, must have a valid driver's license from the employee's state of residence and a valid personal insurance policy on the vehicle which meets the requirements for the state of Tennessee

Licensure, Registration and/or Certification Required by SJCRH Only:

One of the following certifications is required within two (2) years of hire date:

  • Certified Nurse Manager and Leader (CNML) from the American Organizational for Nursing Leadership (AONL)
  • Nurse Executive Certification (NE-BC) from the American Nurses Credentialing Association (ANCC)
  • Nurse Executive Advanced Certification (NEA-BC) from the American Nurses Credentialing Association (ANCC)

Compensation

In recognition of certain U.S. state and municipal pay transparency laws, St. Jude is including a reasonable estimate of the compensation range for this role. This is an estimate offered in good faith and a specific salary offer takes into account factors that are considered in making compensation decisions including but not limited to skill sets, experience and training, licensure and certifications, and other business and organizational needs. It is not typical for an individual to be hired at or near the top of the salary range and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current salary range is $104,000 - $186,160 per year for the role of Manager -Home Health - Community Division.
