We are looking for Denver-based candidates! The Clinical Transformation team is a headquarters team that leads DaVita in the transformation of kidney care by enabling care teams to engage patients in reaching their health goals and reducing overall cost to the healthcare system - otherwise known as value-based care. The Integrated Kidney Care (IKC) Practice Engagement lane is a key sub-lane of Clinical Transformation. This Sr. Manager, IKC Practice Engagement, sits within this sub-lane. Being the Partner of Choice to nephrologist practices allows our care teams to improve the clinical outcomes for our patients and grow this line of business. The IKC Practice Engagement sub-lane seeks to strengthen these practice partnerships through practice collaboration, reporting, technology, innovative care models, etc. This Sr. Manager will drive the strategy, development, implementation, and continuous improvement of innovative initiatives to improve Partner of Choice and associated clinical outcomes - scope may include but is not limited to the following initial focus areas:
The IKC Practice Engagement lane is highly visible and works cross-functionally with stakeholders across the organization, including field operators, clinical, financial, product, and compliance.
