Job Details
Sibley Memorial Hospital in Washington, D.C., a member of Johns Hopkins Medicine, has a distinguished history of serving the community. As a not-for-profit, full-service, 318-bed community hospital, Sibley offers medical, surgical, obstetric, oncology and orthopaedic treatment, intensive care, skilled nursing inpatient services and a state-of-the-art 24-hour emergency department. Sibley's campus is also home to Johns Hopkins Kimmel Cancer Center and The Johns Hopkins National Proton Therapy Center. About this position: In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team. Develops a sustainable, safe transition plan appropriate to the patients' needs and resources in collaboration with the patient/family and interdisciplinary care team. Salary Range: Minimum $41.99/hour - Maximum $65.08/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
