Elwyn California seeks a Mobile Crisis Counselor (MCC) to join the Mobile Assessment Team. This role is responsible for providing mental health support to individuals in locations outside of a traditional office setting. The MCC's function is to work with individuals and their community supports to assist with meeting the individual needs. MCC will also coordinate with outside agencies and community care linkages to support the individual. Mobile counseling services are provided free of charge to Delaware County residents.

DUTIES AND RESPONSIBILITIES:




  • Mobile counselor will work directly with individuals and families to providesupport and brief therapeutic interventions to individuals of all ages in the home or community setting.



  • Facilitate conversations with the family to improve communication and resolve conflicts.



  • Develop and implement strategies to prevent future crises.


  • Utilize a strengths perspective when providing support to the individual and their family.



  • Mobile counselor will receive referrals from the Mobile Crisis Team after crisis intervention forshort term counseling to bridge the gap in on-going services for the individual.


  • Collaborate with all team members in the implementation of treatment interventions and make clinically appropriate referrals and recommendations for effective discharge planning.



  • Foster a strengths based, trauma informed, and accountable relationship with families and the treatment team.


  • Complete accurate and timely clinical documentation.


  • Utilize crisis planning to build on current coping mechanisms.


  • Interface with families of persons served and other interested parties.


  • Intervene in crisis situations.


  • Maintain caseload of 6-8 individuals.


  • The individual needs of the client will influence the duration and frequency of sessions.


  • Attend scheduled staff meetings and appropriate in-service training as directed by supervisor.


  • Provide counseling, guidance, support, resources, and encouragement to persons served, and to take responsibility for and actively participate in their own recovery.


  • Collaborate with the team to promote team culture in which each person's point of view, experiences and preferences are recognized, understood and respected and in which the persons self-determination and decision making in treatment planning are maximized and supported.


  • Increase awareness of, and support participation in, self-help programs and advocacy organizations that promote recovery.


  • Maintain current certification/recertification of CPR and first aid.


  • Perform duties as assigned.



EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:



  • Master's degree required in the field of human services such as Social Work, Psychology or Counseling.


  • Non-profit human service experience highly preferred.



  • Demonstrated ability to work effectively as part of a team.



  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities.


  • Demonstrated strong attention to detail.



  • Demonstrated strong time management and organizational skills.


  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions.


  • Must be able to perform CPR and crisis intervention using agency-trained protocols.


  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication.

  • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint experience preferred.


  • Experience using Electronic Health Record (EHR) / Electronic Medical Record (EMR).

  • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record.
