The Lead Case Manager is designated for the Case Management Department on the Salem campus to oversee daily operations and to work collaboratively with the Case Management Director/ Manager. The Lead CM is responsible for overseeing customer service, serving as a role model for staff, the daily assignment of staff, patient flow and shift to shift decision making regarding staffing. The Lead CM participates with the Director and Manager in the development of the case management standards, evaluation of outcomes and development of improvement initiatives. The Lead Case Manager reports to the Director/ Manager of Case Management.

Compliance Responsibilities: *Understands and adheres to North Shore Medical Centers' (NSMC) compliance standards as they appear in Corporate Compliance Policies, Code of Conduct, and NSMC's Policies. *Keep abreast of all pertinent federal, state and NSMC regulations, laws, and policies as they presently exist and as they change or are modified. Job Specific Responsibilities; (e.g. clerical, patient care, food service, supervisory, etc.) *Assigns Case Managers ; Case Management Assistants and SW and delegates activities as appropriate to cover patient care needs *Coordinates patient care unit coverage to support patient flow activities: admissions, discharges, transfers *Provides support and mentoring to staff- Preceptor for new hires. *Works with the Nursing Manager to plan, coordinate, and anticipate staffing needs *Communicates effectively to all health team members *Serves as a positive professional role model, resource and staff leader *Provides verbal and/or written report to the Director/ Manager on key patient information, upcoming patient flow activity, customer service concerns and unresolved issues requiring follow-up *Acts as role model resource and leader to ensure positive work environment. *Assist CM in handling changes in patient status and monitor for extended observation cases *Supports the CM in handling customer service issues and handles issues directly or refers to Nurse Manager as needed- example - Hospital Issued Notice of Non Coverage *Facilitates effective rounding with the health care team, case manager, M.D. , pharmacy, clinical staff in an effort to promote patient care planning and flow *Coordinates/collaborates with Director of Patient Placement to facilitate patient movement *Will have reduced inpatient assignment on designated Unit with flexibility to participate in trainings; meetings and other duties as required Supervisory/Managerial Responsibilities: *Responsible to oversee day to day function of the Case Management department of the Salem campus - Typical 13 Unit based Case Managers ; 5 Case Management Assistants; 3 Social workers *May give input into annual evaluations Fiscal Responsibilities: *Input into process improvement and department structure to support department goal to match staffing to patient census to meet department budget Job Qualifications: Experience: Licensure, Certifications, or Registration: Physical Requirements
