Full Time 8:00am-4:30pm- Monday-Friday with rotating weekends & holidays

GENERAL SUMMARY

Leads the delivery of quality patient care and adherence to policies, procedures and case management protocols.

Responsibilities:

• Actively participates in the process of monitoring, guiding and evaluating staff performance.
• Assists with coordinating activities, prioritizing workflow, and promoting decision-making and problem solving.
• Performs the duties of a Case Manager.
• Provides consultation for and/or management of patients requiring complex or advanced treatment/care.
