The ideal candidate will possess the following qualifications:

• High school diploma or GED is required.
• 12 credits of higher education coursework in psychology, addictions studies, social work or related field with specialty or concentration in counseling is required.
• One year of case management experience in behavioral health required.
• Preferred, Bachelor's degree in psychology, counseling, human services, or related. PCB credential preferred.

Position Details

The Case Manager is the primary point of contact for assigned consumers, providing individualized, recovery-oriented services and support. This role involves conducting bi-weekly in-person support visits (or as appropriate), developing service and safety plans tailored to client needs, and assisting clients in accessing essential community resources. The Case Manager coordinates care with service providers, monitors client progress, and ensures all documentation is timely and complete, in compliance with agency, county, and state regulations.
