THE POSITION

Cambria County Area Agency on Aging has an exciting opportunity for an individual who is patient, compassionate, and organized. We are looking for an Aging Care Manager that is interested in helping older adults discover resources that will enhance their quality of life. DESCRIPTION OF WORK

As an Aging Care Manager 1, you will participate in a training class where you will gain the skills necessary to effectively assess the needs of older adults, develop care plans, and arrange services to meet their needs. You will be responsible for completing timely recertification assessments for care providers, preparing or helping consumers prepare forms necessary for eligibility determinations, and providing case management services. Work involves linking consumers with appropriate service providers; interpreting and explaining regulations, policies, and agency programs to care providers, consumers, and families; and completing routine monitoring visits to assure regulations compliance. You will also have the opportunity to learn how to perform administrative and counseling duties. This includes protecting client confidentiality, ensuring all applicable consumer data is entered in a consistent and timely manner, and attending meetings. Additional responsibilities include:

  • Providing counseling on available services to consumers, family members, and community

  • Counseling families of consumers on methods of assisting their older family members

  • Providing group counseling as needed

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Three years as an Aging Case Aide 2; or
  • A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
  • Applicants will be considered to have met the educational requirements once they are within # months of graduating with a qualifying degree/credits.

  • Hiring preference for this vacancy may be given to candidates who live within Cambria County. If no eligible candidates who live within Cambria County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

  • A conditional offer of employment will require a medical examination and drug screening.
