1. Manage all intake activities in accordance with MCS policy and procedures to include responding to initial
inquiries within 24 hours; assisting applicants in understanding eligibility and funding options; providing
referrals; and supporting service transitions, as needed. 15%
2. Perform all meetings, tours, and communications in a person-centered manner prioritizing in-person
engagement, when possible. 15%
3. Build and Maintain relationships with local and state agencies for the purpose of connecting with potential
participants for services in MD and VA including: public and non-public schools; Developmental
Disabilities Administration (DDA); DDA Coordination Agencies; Community Services Boards (CSBs);
Department of Behavioral Health and Developmental Services (DBHDS); Division of Rehabilitation
Services (DORS); Department of Aging and Rehabilitation Services (DARS); local DSS; and other
stakeholders. 15%
4. Coordinate and lead Orientation for all new admissions to ensure a successful transition to the applicable
program(s). 10%
5. Document all efforts in the ETO database to ensure applicants are supported in an effective and
comprehensive manner; applicant files are complete and available to applicable programs; and
informtation is tracked to review for improvement regarding service delivery and community engagement. 15%
6. Attend fairs, community events, and workgroups to promote Melwood Community Services professionally
and positively to support the growth of programs and the reputation of Melwood. 10%
7. Provide training to staff in all areas required to support Melwood Community Services participants which
requires becoming a certified train the trainer in CPR/FA and Mandt. 15%
8. Other duties (as assigned): Manage the satisfaction program to ensure services are being delivered is an
effective manner and manage Support Brokerage for Self-Directed Service participants.
