Position Type: Part time, 24 hours/week

Your role with us

As a clinical nurse in advanced wound care, you will be completing comprehensive, age-appropriate assessment and planning to address patients' physical, emotional, and educational needs, particularly related to complex wounds. In the role, you will deliver skilled interventions based on individualized care plans, ensures accurate documentation and communication, and promote safety and quality outcomes through leadership, collaboration, and evidence-based practice. Additionally, you will contribute to effective resource management and support continuity of care across the healthcare continuum. What you'll do

• Performs comprehensive, age-appropriate patient assessments, incorporating medical history, social and cultural factors, and evaluates education needs for patients and families.
• Delivers skilled nursing interventions based on individualized care plans, prioritizing patient needs and proactively preventing complications.
• Develops and adjusts care plans in collaboration with patients, families, and the healthcare team, accounting for rehabilitation potential and discharge planning.
• Accurately documents patient care including assessments, interventions, education, and outcomes to ensure continuity and quality of care.
• Communicates effectively using structured methods like ensuring clear, timely and complete information sharing among team members.
• Demonstrates leadership and collaboration, participating in shared decision-making, mentoring staff, and taking on the charge nurse role as needed.
• Maintains a safe care environment, following infection control and safety protocols while managing supplies and supporting resource efficiency.

• Requires a Bachelor of Science (BSN) or non-nursing bachelor's degree with a Master of Science in nursing (MSN).
• Basic Life Support (BLS).
• Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon.
• One year of nursing experience.

Additional Information

• Acute care RN staff nurse experience preferred.
