Are you passionate about supporting caregivers and older adults in your community? Join Loudoun County's Area Agency on Aging as an Elder Resources Caregiver Case Manager. In this impactful role, you'll provide direct support to family caregivers of older adults and individuals with disabilities, many of whom are experiencing isolation, stress, or difficulty accessing services. You'll offer compassionate case management by helping caregivers find services for both themselves and the people they care for. Responsibilities include conducting home and virtual visits, facilitating support groups and evidence-based counseling programs (such as SHARE), advocating for healthcare and long-term care providers, assisting with Medicaid and SNAP applications, and coordinating a wide range of services, including housing, transportation, and in-home care.

Responsibilities include, but are not limited to: * Provide caregiver-focused case management: Coordinate services for family caregivers of older adults and individuals with disabilities, helping them navigate complex systems and access resources for both them and their care recipients. * Conduct comprehensive assessments: Complete in-home and virtual assessments using the Uniform Assessment Instrument (UAI), along with nutritional screenings and income assessments, to determine eligibility for services and develop individualized care plans. * Support and empower caregivers: Facilitate caregiver support groups, provide one-on-one evidence-based interventions such as the SHARE Program, and offer emotional support and practical guidance to help reduce caregiver stress, burnout, and isolation. * Assist with public benefit applications: Support clients and caregivers in completing applications for Medicaid (including Long-Term Care), SNAP, housing, transportation, and other benefit programs. * Maintain accurate documentation: Develop and manage confidential client records in compliance with grant, program, and County requirements. Submit regular service reports and ensure timely, accurate documentation of all case activities. * Conduct community outreach and education: Design and implement outreach strategies to raise awareness of available caregiver programs and services. * Coordinate services and advocate on behalf of clients: Communicate and collaborate with healthcare providers, discharge planners, assisted living and nursing facilities, home care agencies, and other partners to ensure appropriate service delivery. * May carry out additional critical job responsibilities pertinent to the specific role and departmental assignment.

Minimum Qualifications

Bachelor's degree in Social Work, Psychology, or related field and three (3) years of experience in assigned area or related field; or equivalent combination of education and experience.

Must undergo pre-employment criminal, credit, and DMV background checks. A Valid driver's license and a good driving record are required. Annual reviews of driving records may be conducted to ensure ongoing qualification.
