Summary:

We are excited to offer an opportunity for aHealth Coachto join our GUIDEProgram, a key component of our Dementia Care services. This role is ideal for someone passionate about improving the quality of life for individuals living with dementia and their caregivers.

As a Health Coach, you will provide compassionatepeer support, connect families tocommunity resources, and facilitate access torespite care. Working as part of amultidisciplinary team, you'll play a vital role in enhancing care coordination and helping patients and caregivers navigate the complexities of dementia care.

Ideal Candidate Will Have:

  • A passion for dementia care and supporting caregivers
  • Experience in health coaching, social work, caregiving, or related fields
  • Strong communication, empathy, and problem-solving skills
  • Comfort with remote technology and phone-based care platforms

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree preferred.
  2. License Certification: Valid Driver's License required. Health Coach Certification preferred.
  3. Experience: Two years of experience in a social setting or health care setting or equivalent education preferred.
  4. Additional Skills/Requirements Preferred: Bilingual. Local community resident with knowledge of resources available in the community. Proficient computer use. Knowledge of Medical terminology.
