JOB SUMMARY:
The Clinical Director serves as both a primary care provider and the clinical leader of the City of Frederick's Federally Qualified Health Center (FQHC). This 50/50 hybrid role dedicates approximately half of the time to direct patient care and half to clinical leadership, oversight, and compliance.
The Clinical Director delivers high-quality primary care to low-income and homeless patients eligible for services through the Department of Housing and Human Services. Care is provided in an outpatient, ambulatory setting and includes preventive screenings, chronic disease management, patient education, diagnosis and treatment, medical documentation, referrals, and care coordination.
In a leadership capacity, the Clinical Director provides clinical supervision (if MD/DO) to nurse practitioners and physician assistants, develops and implements clinical protocols and policies, and ensures compliance with HRSA Health Center Program requirements. They lead quality improvement efforts, consult on complex cases, and help integrate services across the Health Center. This position also supports the day-to-day operation of the clinic and advances the Health Center's mission to deliver trauma-informed, equitable, and community-based care to Frederick's most vulnerable residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Responsibilities (Approx. 50%)
- Provides direct patient care including primary care, health assessments, intake and medical history, medical screening, preventive services, first aid, and health education, with a focus on low-income and homeless populations.
- Delivers care in an outpatient, ambulatory setting and through outreach-based services for individuals who are unable or unwilling to access care at the Health Center.
- Implements health care screening programs targeting chronic conditions, infectious disease, and common issues associated with homelessness and poverty.
- Provides referrals to appropriate medical, behavioral health, and addiction treatment services.
- Develops and delivers targeted health education sessions in clinical and community settings.
- Maintains comprehensive and timely documentation in electronic health records (EHR) and paper files, in compliance with all clinical and regulatory standards.
- Provides medical on-call services on a rotating basis.
- Supports and guides the clinical experience of nursing and medical students on site at the Health Center.
- Conducts home visits, street outreach, and shelter-based care as needed to engage vulnerable individuals in services.
- Facilitates communication and coordination with external outreach sites, shelters, and partnering agencies.
- Responds to environmental or clinical safety incidents and assists in maintaining the cleanliness and clinical readiness of the Health Center.
Leadership, Oversight & Administrative Responsibilities (Approx. 50%)
- Provides clinical supervision and support to mid-level practitioners (NPs/PAs), ensuring compliance with standards of care, licensure, and scope of practice.
- Develops, implements, and periodically reviews medical protocols, policies, and procedures in alignment with HRSA Health Center Program requirements.
- Leads quality improvement initiatives, peer review activities, and efforts to improve patient outcomes and clinical efficiency.
- Serves as a medical consultant to other providers and oversees coordination of care for complex patients.
- Collaborates with the Assistant Director of Community Health Services on clinic operations, provider productivity, and patient flow.
- Participates in strategic planning, budget discussions, and staffing model development to support long-term growth of the Health Center.
- Represents the Health Center in local and regional health partnerships, including with hospitals, behavioral health providers, and public health agencies.
- Supports recruitment, credentialing, and onboarding of clinical staff.
- Maintains all required State licensure and Board certification relevant to the scope of practice.
- Provides in-service education to clinical and interdisciplinary staff on relevant public health topics or best practices.
- Perform other duties as assigned to support the Health Center's mission and programs.

REQUIRED EDUCATION AND/OR EXPERIENCE:
- Medical Doctor (MD/DO) or Advanced Practice Registered Nurse (APRN/NP) with a graduate degree in a primary care discipline is required.
- For MD/DO: Graduation from an accredited medical school and successful completion of a residency in a primary care field (e.g., Family Medicine, Internal Medicine, Pediatrics, OB/GYN, or Psychiatry).
- For Nurse Practitioners: Graduation from an accredited nurse practitioner program with national board certification in a primary care specialty.
REQUIRED CERTIFICATES AND/OR LICENSES:
- Current licensure to practice medicine in Maryland.
- Current licensure and Board-certification as required by the State of Maryland, Board of Physicians to practice Internal Medicine, Family Practice, Pediatrics, OB/GYN or Psychiatry.
- Vehicle Operator's license with satisfactory driving record that meets insurability standards of the City's insurance carrier is required. The employee must maintain the license for the duration of their employment.
REQUIRED SKILLS (Communication/Language; Mathematical; Computer; etc.):
- Good clinical skills in the areas of patient assessment, history taking, health education, and treatment.
- Ability to read and interpret documents using the English language.
- Strong interpersonal communication skills and an ability to provide services to people who are low-income and/or homeless.
- Good written and oral communication skills.
- Ability to use basic computer programs such as Microsoft Word and Access.
- Ability to use or learn to use the Electronic Medical Records programs used by the Department of Housing and Human Services
OTHER DESIRABLES:
- Ability to communicate using the Spanish Language.
- Ability to communicate using American Sign Language.
- Background in primary care to underserved populations.
