Compensation

Salary Range: $22.56 - $32.31 / hour

The pay range above represents the lowest possible rate for the position and the highest possible rate. Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role. Most often, a newly hired employee will be placed below the midpoint of the range.

Pay for PRN positions is calculated differently than Full-Time, Regular Part-Time, and Part-Time positions. PRN Rates depend on the position and other factors, including years of experience, or shift commitment according to the department policy. PRN differential is added in certain positions.



Responsibilities

The Case Management Associate is responsible for keeping official health system records and executes administrative policies determined by, or in conjunction with, other officials.



Qualifications

Minimum Qualifications:

    High school diploma or equivalent (GED)
  • One (1) year of experience in Microsoft Office, including Word, Excel and Outlook

Preferred Qualifications:

  • Associate's degree (A.A.)
  • One (1) year of administrative experience in a healthcare or social services setting
  • One (1) year of customer service in a healthcare or a related setting, with a proven ability to address patient inquires and concerns effectively
  • Proficient in Microsoft Office

Essential Functions and Responsibilities:

  1. Administers to all Medicare patients a second written Notification of Hospital Discharge Appeal Rights, as mandated by the Centers for Medicare and Medicaid Service (CMS) guidelines.
  2. Addresses patient/family questions regarding the Notification of Hospital Discharge Appeal Rights and refers any additional questions or concerns appropriately.
  3. Establishes a tracking system for patients requiring a second notice under the CMS guidelines.
  4. Establishes and maintains records as they pertain to the above.
  5. Assists Case Managers with arranging post discharge services, including follow-up on referrals and photocopying medical records (hard copy or onto a disc) upon request.
  6. Interacts effectively with adult and senior patient populations from diverse cultures and with all levels of staff and medical personnel.
  7. Writes various letters and reports.
  8. Performs other duties as assigned.
