Job Description
General Purpose of Job: Responsible for providing high-quality, patient-centered care through assessment, evaluation, diagnosis, treatment and coordination of medical services under the supervision or collaboration of a physician. This includes acute care, chronic disease management, admissions, consultations, and discharges across specialties. Essential Duties:
- Duties listed below reflect the core responsibilities of this role, but additional tasks may be assigned based on clinical or department needs.
- Facilitates appropriate admissions, consultations, transfers, and discharges to health system service lines by initiating orders, coordinating with the attending physician, and overseeing the medication reconciliation process.
- Performs and documents history & physicals, consultations, progress notes and discharge summaries in accordance with institutional standards.
- Collaborates with attending physicians, consultants, nursing staff and interdisciplinary teams to develop and modify care plans.
- Orders and interprets diagnostic tests including laboratory studies, imaging and specialty-specific diagnostics to support clinical assessment and treatment planning in collaboration with treatment team.
- May perform clinical procedures appropriate to training, scope of practice, and credentialing under the supervision or delegation by the attending physician.
- Participates in procedure related education, training and competency assessments required by the service line.
- Maintains knowledge of safety protocols and documentation standards for all procedures performed.
- Participates in daily rounds and interdisciplinary meetings as required by the service line.
- Provides timely patient and family education and communicates care plan updates clearly and effectively.
- Performs other duties as assigned in alignment with scope of practice and organizational needs.

Job Requirements
Work Experience |
* One year experience as a Nurse Practitioner or Physician Assistant in a healthcare setting preferred. * New graduate Nurse Practitioners and Physician Assistants are welcome to apply - strong mentorship and onboarding support provided. * Familiarity with electronic health records (EHR). |
Education |
* Master's degree in Nursing (MSN) or graduate of an accredited PA Program required. * Current Michigan RN license with Nurse Practitioner certification or Physician Assistant in the State of Michigan required. * National certification in respective role required. * BLS certification is required. Additional certifications such as ACLS, PALS or specialty specific credentials may be required based on the clinical service line. |
Specialized Knowledge and Skills |
