Summary of Job Duties:The Primary Care Behavioral Health staff will serve as part of the Patient Centered Medical Home and work within an integrated treatment setting using a behaviorist/consultant role. The PCBH will provide direct care to clinic patients, as referred by physicians and work with the treatment team to promote patient health outcomes. Qualifications:

  • Master Degree in Social Work
  • Arkansas Licensure - LMSW/LCSW

Job Requirements:

  • Provide education to providers in BH to support identification and treatment of mild to moderate symptom acuity. Assist in educating providers in BH assessment methods, interpretation and patient intervention strategies for use with primary care setting. If applicable, develop and implement BH residency training curriculum.
  • Complete documentation for all patient interactions within EPIC using SW visit type. Review clinic appts for patients with BH conditions to help identify and communicate patient BH needs to primary care physicians.
  • Using a Behavioral Health Consultant model, work with clinic team to provide brief, evidence based interventions to clinic patients referred by primary care provider. Participate in morning huddles, case conferences, complex care team and provider meetings to advocate and promote patient care and outcomes for individuals with behavioral health conditions.
  • Attendance at clinic meetings, participate in practice improvement programs, staff trainings. If applicable, participate in residency interview. Role model collaborative team minded working relationships in both clinic and academic responsibilities.
  • Provide feedback to primary care physicians on a same day basis for all initial patient visits.
  • Perform other duties as needed.
