Job Summary: The Program Support Specialist plays a critical role in ensuring the accurate and timely processing of patient referrals and program documentation. This position requires strong attention to detail, excellent communication skills, and the ability to work collaboratively with internal teams and external stakeholders. The specialist will manage patient information, triage requests, and support case managers in delivering high-quality service aligned with program guidelines.
Key Responsibilities: * *Form Review & Data Entry* * Review incoming forms for completeness and legibility. * Follow up with stakeholders to obtain missing or unclear information. * Accurately capture program-specific data into the designated software platform. * *Referral & Profile Management* * Process referrals in accordance with program timelines and requirements. * Create new patient profiles or correctly associate documents with existing profiles. * Assess requests and assign appropriate services/resources based on program rules. * *Compliance & Reporting* * Identify and report pharmacovigilance information (e.g., Adverse Events) as required by clients. * Maintain confidentiality of patient and proprietary information at all times. * *Communication & Coordination* * Answer and triage inbound calls to case managers or relevant program partners. * Make outbound calls to patients for refill reminders and follow-ups. * Triage referrals and assign tasks to case managers based on next-step protocols. * *Team Collaboration* * Work closely with internal teams to resolve issues and ensure smooth workflow. * Support program-specific initiatives and contribute to continuous improvement efforts. Qualifications: * High school diploma or equivalent; post-secondary education preferred. * Experience in healthcare, customer service, or administrative support roles. * Strong verbal and written communication skills. * Proficiency in data entry and use of software platforms. * Ability to manage multiple tasks and prioritize effectively. * Knowledge of patient confidentiality and data protection standards. Preferred Skills: * Familiarity with pharmacovigilance and healthcare compliance. * Experience working in a referral or case management environment. * Bilingual capabilities (if applicable to program needs). *Pay and Benefits* The pay range for this position is $17.00 - $18.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
