Outreach Coordinator

We are searching for an individual to fill an Outreach Coordinator position for the Family Access Center for Excellence (FACE) of Boone County, Missouri. FACE Community-Based Services is a community access center that provides evidence-based clinical case management funded by the Boone County Children Services Fund. It is affiliated with the University of Missouri's Department of Educational, School and Counseling Psychology, the Missouri Prevention Science Institute, the Department of Psychological Sciences, and the School of Social Work. FACE is an innovative approach to coordinating and improving the mental and physical health, well-being, and safety of children and families in Boone County, MO. FACE evidence-based services promote and empower family involvement and choice throughout the creation and progress towards case management goals.

Job Description

Outreach Coordinator will:

  • Utilizes the evidence-based practice of Motivational Interviewing while engaging families across the community. Contacts families to share about our services and schedule an appointment with those interested in case management services.
  • Participates in ongoing community level meetings with other agencies and/or service providers to increase collaborative efforts. Develops working relationships with various sectors (schools, law enforcement, etc.) and community agencies / service providers to raise awareness regarding our services and increase referrals received.
  • Assists in the development and distribution of outreach materials and PowerPoints for community tabling events, presentations, etc. Participates in tabling events across the community and provides presentations to sectors and community agencies re: our services and referral processes.
  • Maintains documentation in an electronic medical record (EMR) re: client demographics, referrer information, appointment scheduling, families presenting concerns, and other pertinent data and information. Participates in ongoing internal agency meetings, case consultations, and supervision.
  • Connects families experiencing a safety concern or crisis to a trained co-worker that can provide the level of support needed. Creates menus of service options to provide families and community service providers based on concerns, preferences, insurance, financial situation, and other factors.
  • Participates in monthly all staff meetings, weekly team meetings, and supervision that occur in-person at the office and/or via telehealth
Shift

The Outreach Coordinator position is a full-time, remote position. FACE hours of operation are Monday - Thursday 9 a.m. to 7 p.m. and Friday 9 a.m. to 5 p.m. Position includes some evening and/or weekend hours to participate in community events and meetings across Boone County as they occur. Position includes attending in-person monthly and weekly agency meetings at the office. A portion of working hours may occur remotely at the employee's residence, thus the position includes the ability to have a confidential space at home in which to work.

Minimum Qualifications

Bachelor's degree or equivalent combination of education and experience from which comparable knowledge and skills can be acquired is necessary.

Preferred Qualifications

Prior experience working with various populations is preferred. Candidates will be evaluated on the following factors:

  • Ability to provide supportive experiences for families and community referrers in-person or via telehealth
  • Ability to implement and coordinate outreach activities across Boone County such as providing community presentations and/or participating in tabling events while effectively communicating the agency mission and services
  • Ability to collaborate across multiple community disciplines and sectors such as school personnel, law enforcement, medical providers, etc.
  • Ability to utilize an electronic medical record to register and schedule families for appointments and complete documentation
  • Communication, organizational, and management skills
  • Ability to provide service to the program, department, college, and the profession

Anticipated Hiring Range

Salary Range: $18.87 - $22.00 per hour
