Overview

We are seeking a results-driven and experienced clinical leader to drive provider education, quality improvement, and operational excellence across Abode Care Partners. This individual will play a key role in developing scalable training programs, standardizing clinical workflows, and enhancing documentation and coding practices that advance value-based care initiatives. The ideal candidate combines deep clinical expertise with proven experience in operational management, workforce development, and large-scale healthcare integration.



Responsibilities

    Clinical Training & Development: Design and deliver comprehensive clinical education programs for ACO partners and Abode providers through virtual and in-person modalities.
  • Performance Improvement: Conduct chart reviews and create scalable feedback loops that improve provider performance, documentation accuracy, and clinical quality outcomes.
  • Workflow Optimization: Collaborate with operations teams to establish standardized clinical workflows, EMR templates, and best practices that enhance efficiency and compliance.
  • Policy & Guideline Management: Author and maintain clinical policies, procedures, and documentation standards, including HCC and risk adjustment best practices.
  • Onboarding Leadership: Lead onboarding programs for new providers, including coordination of site visits, training sessions, and competency evaluations.
  • Operational Excellence: Design and implement operational workflows supporting direct patient care and ancillary services to ensure seamless care coordination and efficiency.
  • Strategic Expansion & Integration: Partner with executive leadership to support market integrations, workforce scaling, and expansion initiatives across multiple geographies.
  • Cross-Functional Collaboration: Work closely with clinical, operational, and technical teams to ensure alignment between strategic goals, provider engagement, and patient outcomes.


Qualifications

  • Bachelor's degree required.
  • Minimum of 5 years of clinical experience, including at least 2 years in a leadership, quality, or education role.
  • Proven success leading teams, scaling operations, or integrating healthcare markets.
  • Experience with value-based care models, ACOs, or population health preferred.
  • Proficiency in EMR systems (e.g., Athena) and clinical documentation standards.
  • NP, PA, RN, or equivalent clinical background required.
  • Experience developing clinical education content and leading provider training initiatives.
  • Strong knowledge of coding and documentation (e.g., HCC, RAF, risk adjustment).
  • Demonstrated ability to lead cross-functional teams through large-scale operational transformations.
  • Background in telemedicine, P&L management, or multi-market healthcare integration a plus.
  • Ability to translate clinical strategy into measurable outcomes, including quality, engagement, and cost metrics.
  • Exceptional leadership and team-building skills with the ability to recruit, mentor, and retain top clinical talent.
  • Strong communication and interpersonal skills to engage effectively with physicians, staff, executives, and external partners.
  • Advanced skills in data-driven decision making, including analysis of clinical and operational metrics.
  • Proficiency in developing and delivering education and training programs for diverse clinical audiences.
  • Negotiation and contract management skills to navigate partnerships with SNF chains and healthcare organizations.
  • Change management skills to drive adoption of new technologies, platforms, and documentation standards.
  • Comprehensive knowledge of clinical best practices, regulatory standards, and healthcare compliance requirements.
  • Strong understanding of quality assurance, performance improvement, and value-based care models.
  • Knowledge of credentialing processes, medical staff governance, and contracting requirements.
  • Familiarity with healthcare business operations, strategic partnerships, and financial principles.
  • Ability to balance clinical excellence with organizational growth and operational priorities.
  • Ability to collaborate and influence across interdisciplinary teams and at all leadership levels.
  • Ability to lead system-wide initiatives and drive measurable improvements in quality, compliance, and efficiency.
  • Ability to think strategically and translate vision into actionable clinical and business initiatives.
  • Ability to foster a culture of accountability, continuous learning, and high performance.
  • Travel 25-75%
