Lead and inspire teams to improve patient experience and retention across all Home modalities (e.g., peritoneal dialysis, home hemodialysis). This role requires a visionary leader who can set direction, foster a culture of accountability, and drive results through collaboration with clinical, operational, and educational partners. The Director will champion cross-functional initiatives, pilot innovative programs, and ensure best practices are adopted organization-wide to achieve ambitious retention goals.

1.

Support retention manager pilot effort, specifically:

  • Train retention managers on how to focus time and effort on high impact patients in divisions below mean quality outcomes for the 4 KPIs across all geographies
  • Leverage and drive adoption of innovative IT programs, including AI predictive models, home remote monitoring, clinical management tool and virtual/population health tools
  • In conjunction with other Home clinical leaders , provide subject matter expertise to drive improvement in clinical management
  • Assist in the development of processes to implement known best practices, learn, and create accountability to processes and monitoring of results
  • Identify physician and team barriers to success and actively address/remove, leveraging physician and clinical SME support as appropriate
  • Share learnings from pilot and drive implementation of learnings across the Village

Provide consistent clinical oversight and direction to focused home national activities and create integration with broader clinical experts across the Village.

  • Act as advisor to development team for an AI-based model to predict patients at highest risk of loss and develop interventions to maximize clinically appropriate time on therapy
  • Provide subject matter expertise and training to support deployment of technology and processes known to reduce loss and partner with field teams to make division leadership more accountable for clinical results.
  • Identify and prioritize potential best practices requiring further research or proof through collaboration with Clinical Enterprise, Pioneer and DaVita Clinical Research; Evaluate results and create standardization of practices that have proven impact

Develop a data-driven standardized approach for loss prevention in all Palmers

  • Support efforts to educate and drive standardization of practices with OPUS and our Home Directors of Clinical Services across the Village, in partnership with our Senior Director of Clinical Services for Home and our Chief Medical Officer of home
  • Partner with OPUS and CE to upskill CSS and DCS on known best practices; create accountability to results
  • Assist in the development of loss prevention clinical information utilized by Clinical Education for preceptor and new hire curriculums
  • Partner with Clinical Education to ensure that continuing education and in-services/materials to Home teammates are aligned with most critical insights and practices that drive patient retention
  • Partner with Home Growth Manager to ensure that vendors and external clinical support systems are aligned with Village retention priorities to maximize impact

This position will be open for a minimum of three days.

The Salary Range for the role is $114,000.00 - $181,000.00/year. If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position. New York Exempt: New York City and Long Island: $64,350.00/year, Nassau, Suffolk, and Westchester counties: $64,350.00/year, Remainder of New York state: $60,405.80/year New York Non-exempt: New York City and Long Island: $16.50/hour, Nassau, Suffolk, and Westchester counties: $16.50/hour, Remainder of New York state: $15.50/hour Washington Exempt: $77,968.80/year Washington Non-exempt: Bellingham: $17.66/hour, Burien: $21.16/hour, Unincorporated King County: $20.29/hour, Renton: $20.90/hour, Seattle: $20.76/hour, Tukwila: $21.10/hour, Remainder of Washington state: $16.66/hour
