Position Summary: The Case Manager (CM) works as a member of a multi-disciplinary team within VOA Oregon's outpatient and residential substance use disorder and mental health treatment programs providing case management services to clients. The CM provides direct support to clients to include systems navigation, needs assessment, linkages to community resources including housing, financial, medical, employment, benefits, and other services as needed.

Education, Training and Experience:



  • Bachelor's degree in psychology, social work, or related behavioral health field or a minimum of 3 full years of mental health-related experience.
  • Requires QMHA credentialing through Mental Health & Addiction Certification Board of Oregon (MHACBO) upon hire.
  • Certified Alcohol and Drug Counselor certification preferred.
  • Experience working with a culturally diverse population.

  • Knowledge of motivational interviewing, assessing needs, skill building, and group facilitation skills.
  • Ability to write accurate, concise reports using electronic medical record program while meeting tight deadlines.
  • Knowledge and skills to deliver culturally competent services and work effectively in multicultural and diverse socioeconomic situations.
  • Mission focused and passionate, with an emphasis on serving high-risk populations in evidence-based practices.
  • Commitment to and actions consistent with, the highest level of ethical and professional standards.
  • Excellent communication, information retrieval, and dissemination, implementation, and relationship-building skills.
  • Organizational, and time management skills.
  • Proficient in Microsoft Office and electronic health records (EHR's).


Additional Eligibility Requirements:



  • Reliable form of transportation.
  • Ability to pass a background check.


Essential Duties and Responsibilities:



  • Provide housing assistance including tenant education, advocacy, and connection to housing resources.
  • Assist with medical and dental resource coordination and connecting client to needed services such as NAMI, AA, NA, etc.
  • Assist client to obtain food stamps, food boxes, etc. and accessing transportation resources.
  • Assist client in navigating application processes.
  • Coordinate the resources needed with probation services and other treatment providers
  • Aid in obtaining entitlements like SNAP benefits and/or social security
  • Maintain case files and ensure all required paperwork is kept up to date.
  • Establish service linkages and develop and maintain positive relationships with community groups, social service agencies, and other community partners.
  • Work collaboratively with staff across Outpatient Services, providing education and co-case management support as needed.


Other Responsibilities:



  • Other duties as assigned.


Schedule: This is a full-time position; 40 hrs. per week; Monday thru Friday, with occasional evenings and weekends as needed.
