Under general supervision, and in addition to performing the full range of operational-level duties, the Clinical Social Worker is responsible for administering licensed social service programs or relevant work. The Social Worker is responsible for performing psychosocial assessments, assisting the palliative care team, and/or directly facilitating patient-family meetings for goal setting and advanced care planning. The Social Worker provides emotional support for patients and families facing advanced illness, and coordinates educational and outreach services relevant to the social service-palliative care interface. The Social Worker provides licensed social work services consisting of discharge planning, continuing care services and clinical social work services, including crisis intervention, mandated reporting, information and referral, and coordination of health and mental health services with community providers. The social worker is responsible for the education and advocacy of patients, families and/or significant others. The Social worker participates in related department functions, such as community and program education. Complete assessments within two days of consult, completes weekly and discharge progress notes, and has the ability to case find. Performs all other duties as assigned. MINIMUM QUALIFICATIONS
Pay Transparency Act Annual Full Pay Range: $100,408 - $132,296 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $48.09 - $63.36 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable). | |
