Job Title: Program Manager | Rehabilitation and Post-Acute Care

Location: Hybrid - Expected on-site presence in La Crosse 1-2x/week on average. Eligible candidates must reside within a reasonable distance to La Crosse, WI.

Position Overview: The Program Manager provides strategic leadership for the development and improvement of Rehab and Post-Acute Care services across the Emplify Health Gundersen region. This role ensures high-quality, compliant care delivery and fosters collaboration across multiple care settings.

Key Responsibilities:

  • Provide local and regional strategic and tactical leadership to achieve organizational goals related to all administrative and clinical components of Rehab and Post-Acute Care programs.

  • Develop, implement, and evaluate program services aligned with Emplify Health's mission and strategic plan.

  • Actively represent Emplify Health at state and national initiatives to advance program objectives.

  • Manage budgets, control costs, and identify opportunities for financial efficiency while maintaining high-quality patient care standards.

  • Collect, analyze, and report data related to outcomes, utilization, costs, and reimbursement (including Prospective Payment System [PPS] data) to inform leadership and stakeholders.

  • Oversee denials management in collaboration with the compliance department.

  • Recommend, develop, innovate, and market services and programs that support patient care and accreditation standards (CARF & Joint Commission).

  • Design, implement, and oversee Post-Acute Care programs to ensure quality and efficiency throughout the patient care journey.

  • Assist geriatric clinicians in standardizing workflows within nursing homes and assisted living facilities to improve clinical efficiency and care standards.

  • Build and maintain strong relationships with internal departments (social services, nursing, ancillary support, clinical teams) and external partners (healthcare providers, hospitals, rehab centers, nursing homes, home health agencies) to optimize patient care pathways and ensure seamless transitions.

  • Develop educational and supportive materials to engage patients and families in understanding Post-Acute Care options, care plans, and resources.

  • Maintain current knowledge of Medicare and Medicaid payment rules, policies, and procedures related to data collection, submission, and compliance.

  • Travel within La Crosse County and the tri-state area to support facility partnerships and program growth.

Qualifications:

  • Bachelor's degree in a health-related field required; Master's preferred.

  • 3-4 years of progressive healthcare experience.

  • Strong leadership, project management, and relationship-building skills.

  • Detail-oriented with experience in regulated healthcare environments.

  • Comfortable with interpersonal interaction and travel.
