Organization: Lutheran Social Services of WI and Upper MI Are you passionate about making a difference in the lives of children and families? Lutheran Social Services is seeking a Care Coordinator to support families through the Children's Long-Term Support (CLTS) Waiver Program-a federally funded Medicaid program serving children with developmental, physical, or severe emotional disabilities. As a CLTS Care Coordinator, you'll be the main point of contact for families, helping them access services that keep children safe and thriving in their home and community. What You'll Do
