SUMMARY:
The Social Worker is responsible for providing high-quality patient care, displaying good skills in assessment, psycho diagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. Additionally, he/she is expected to function well as a member of the multidisciplinary team and participate in the unit community relations program. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES: KEY RESPONSIBILITIES: Ensures patient dignity and respects patient rights and values. Operates within ethical and compliance standards. Ensures all regulatory standards and requirements are met (DMH, DPH, CMS, Joint Commission etc.). Participates in the assessment of potential admissions to Geri Psych Unit, facilitating decision in timely manner. Able to assess and reassess the needs of the patient. Properly documents assessments. Able to communicate the needs of patients. Completes psychosocial evaluations on assigned patients in a timely fashion. Displays good interviewing, diagnostic, and report-writing skills. Conducts therapies in accordance with patient diagnosis and needs. Provides efficient discharge planning throughout treatment process. Completes progress notes and assessments according to program procedures. Completes individualized treatment plans for each patient. An active member of the interdisciplinary team. Conducts individual psychotherapy sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of psychopathology and good treatment technique. Conducts family therapy sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of family therapy principle and good treatment technique. Conducts group therapy sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of group therapy principles and good group therapy technique. Engages in timely, efficient discharge planning. Works in close conjunction with patient, family, nursing staff, and attending physician. Shows good knowledge of referral/placement process and available community resources. Functions cooperatively as a member of the treatment team. Provides ongoing education to patient and family as needed. Conducts educational group sessions, displaying good theoretical knowledge of subject matter and effective communication skills in presenting material. Communicates effectively with families and agencies, conveying a positive, professional image. Provides resources and support for patient before, during, and after treatment of patient. Actively involved in measuring patient performance. Communicate results and trends to supervisor. Provides feedback to supervisor on ongoing basis in regards to concerns, improvements, changes, etc. Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as required. Brings to the Program Director's attention significant issues related to program functioning. Utilizes feedback regarding performance and uses supervision to improve performance. Ensures facility is safe and reports deficiencies to supervisor. Completes all required competencies per program regulations. Attends all in-services, seminars, etc. to promote learning. Keeps current on CEU requirements to maintain licensure status. Completes work assignments within an acceptable time frame. Uses time and resources to the best possible advantage for successful completion of job responsibilities. Develops and maintains efficient working relationships. Is reliable in respect to attendance and attendance. Recognizes the importance of team efforts and partners with others to achieve positive outcomes. Is part of a team effort to ensure quality services. Offers creative solutions or alternatives to issues or concerns. Produces quality results. Continually strives to improve the quality of work. MINIMUM QUALIFICATIONS: REQUIRED QUALIFICATIONS: Two years' experience in the clinical psychiatric setting required (5 years preferred), with prior experience in Geriatric Psychiatry preferred. Excellent interpersonal and organizational skills with the ability to function in a fast-paced environment Understanding of Joint Commission standards related to the Continuum of Care; Patient Rights and Organizational Ethics; Education of Patients and Family; Management of Information; Infection Surveillance; Prevention and Control; Care of Patients; Assessment of Patients and Improving Organizational Performance. Excellent assessment, diagnostic, and crisis intervention skills. Working knowledge of Utilization, Case Coordination, Risk Management and PM/PI Experience in developing and facilitating patient discharges from an acute care setting Ability to complete a report of study findings Manage stress appropriately and make decisions under pressure. Manage the anger/fear/hostility/violence of others appropriately. Must be able to work independently and collaborate as part of the program care team. Ability to report conclusions, analyze data and write an action plan to address recommendations Excellent written and oral communication skills with a demonstrated ability to communicate effectively Is cooperative in interactions, treating customers with courtesy, respect and compassion. Is patient when responding to questions and answers questions appropriately. Responds to requests in a timely manner. Handles interruptions in a skillful way. Maintains a professional appearance and demeanor. Strives to prevent/resolve customer concerns to the customer's satisfaction Involves other departments in project planning and implementation. Supports others in the department and provides help and assistance when possible. Regularly attends and participates in team and/or departmental meetings and activities. EDUCATION: Master's in Social Work Current Licensure in Massachusetts as Licensed Independent Clinical Social Worker (LICSW). Evidence of professional development.
