Customer Service experience is a plus!

The ideal Discharge Planner candidate will oversee discharge planning process, utlizing the strength of both Case Management and Social Services to ensure the appropriate resources and services are available and arranged for our patients. Communicates with third party payor case management/ insurance companues and provides documentation/ communication as required. Develops and maintains a marketing network with professionals in the community.
