Position Summary:
We have an exciting opportunity to join our team as a Counselor.
In this role, the successful candidate The counselor for the Virtual Primary Care project is responsible for screening patients for program eligibility and social drivers of health, providing education about TytoCare to eligible patients, addressing identified social drivers of health, and assisting patients in accessing virtual health services.
Job Responsibilities:
- Conducts assessment to determine program eligibility
- Screens for SDOH and addresses identified needs
- Provided education and information about Virtual Primary Care project and TytoCare
- Provides assistance to patients in using MyChart and in uploading any required documentation
- Assists patients during virtual visits as needed
- Screens patients for eligibility for resources and benefits.
- Assists clients with benefit applications
- Makes referrals to a variety of public and community agencies providing food, shelter, clothing, medical, educational, and other services.
- Assist community members in accessing health and behavioral health services within the FHC
- Complete documentation in EPIC as required.
- Attend staff meetings and other required meetings.
- Counsels clients on available resources and provides advocacy
- Strengthens relationships with community partners to identify resources for clients
- Attends required meetings
Minimum Qualifications:
To qualify you must have a High School Diploma or equivalent required.
Bachelor's Degree preferred.
1+ years relevant experience working, ability to plan and manage,
Knowledge of public benefits
History of immigrant community engagement
Preferred Qualifications:
Excellent communication skills, ability to multi-task, experience in providing services to diverse community, ability to be a team player.
Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,137.15 - $48,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
