The coordinator will also play a key role in data collection and analysis to advocate for further expansion of community-based programs and services to meet the growing needs of this population. DESCRIPTION OF WORK

The Complex Case Dual Diagnosis Cross-System Coordinator will serve both the adult and children/youth populations, supporting planning, coordination, and service delivery efforts for consumers identified as Seriously Mental Ill (SMI) and dually diagnosed with an intellectual disability or Autism (IDA) diagnosis. This position focuses on individuals who are involved in multiple systems (e.g., Mental Health, Developmental Services, Physical Health) and experience complex needs across these systems. The Dual Diagnosis Cross-System Coordinator will act as a central point of contact, ensuring comprehensive case coordination and addressing needs proactively. This role will foster collaboration with various program offices within the Department of Human Services (DHS) and external community partners to streamline care, reduce system gaps, and prevent cases from escalating to a higher level of complexity.

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.
  • Occasional need to work or attend meetings after hours or on weekends. Compensation provided.
  • Telework: You may have the opportunity to work from home (telework) part-time. Employee will be eligible for up to three days of telework per week after successful completion of a six month probationary period and director approval. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Greensburg. The ability to telework is subject to change at any time. Additional details may be provided during the interview.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of professional experience in the field of intellectual disabilities, and a bachelor's degree; or
  • An equivalent combination of experience and training

  • Hiring preference for this vacancy may be given to candidates who live within Westmoreland County. If no eligible candidates who live within Westmoreland County apply for this position, candidates who reside in other counties may be considered.
