Title: Community Health Worker Duration: 6+ months Contract Pay rate: $26/hr w2 Location: Houston, TX 77036 Shift timings: Mon - Fri 8 AM - 5 PM BG/DT required: Yes Job Description: Responsible for diverse outreach, education and community coordination activities such as canvassing local businesses/organizations, attending community events, conducting outreach phone calls, and giving presentations. Serve as a subject-matter expert to meet outreach objectives for HCPH programs and recommend changes to improve outreach strategies and support the creation/updating of HCPH program trainings for the entire outreach team. Maintain knowledge/skills to educate, promote, and connect community members to all HCPH programs/services. Track, collect, and submit outreach service data on a daily and weekly basis. Responsible for initiating referrals to programs/services, while ensuring community members' confidentiality as well as building and maintaining relationships with stakeholders and organizations to create and sustain innovative partnerships. Create and deliver educational presentations/workshops on public health topics for diverse target audiences and other duties as assigned, including special tasks involved with responding to emergency events. Minimum Requirements: High School Diploma. Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Physical Requirements: This is a largely stationary role, however does require field work and many duties would require the ability to lift up to 20 pounds and bending and standing for periods at a time.
