Registered Nurse Case Manager (Full-Time, 37.50 hours per week) Are you looking for a better work/life balance? No evenings, weekends, holidays, or on-call. Responsibilities include determining clinical eligibility for Mass Health-funded services for nursing facility stays and completing in-home assessments of service needs for individuals. Excellent written, communication, and computer skills required. Geriatric/Long Term Care and community-based nursing experience preferred. Registered nurse with a valid Massachusetts license. Bachelor of Science degree in Nursing or Associate of Science in nursing and at least one year of clinical experience in a community or long-term care setting. Qualifications Ability to exercise independent judgment Must possess and demonstrate critical thinking skills Demonstrated interpersonal skills Ability to develop and maintain records and communicate clinical data in verbal and written form Demonstrated knowledge of the principles and practices of nursing Valid Driver's License (and insurance) Required Pay Range - $32.88 - $33.39 per hour Day Shift - Monday - Friday Work Location - Berkshire County - travel reimbursement For additional information about Elder Services, please visit our website at esbci.org.
