Responsible for delivery of nutrition care to transplant patients and live organ donors in outpatient settings. Provides individual and group medical nutrition therapy to a wide variety of patients across multiple diagnoses including but not limited to: diabetes, hypertension, lipid disorders, obesity, pre and post transplant as well as living donors, GI disorders, eating disorders and others. Able to triage and counsel patients, and document care in the medical record. Attends all department and Transplant Center meetings as required. Develops, implements, and evaluates outcome information and education materials on an on-going basis.
Pre-transplant Evaluation: The Transplant Clinical Dietitian - Outpatient will complete a nutritional assessment for each potential transplant recipient. Live organ donors will receive nutrition assessment as referred. The outcome of this evaluation will be communicated to the Transplant multidisciplinary team and documented in the patient's medical record. The evaluation may include:
Selection/Care Planning: A Transplant Clinical Dietitian - Outpatient will participate in multidisciplinary care and selection of transplant recipients/donors, including participation in Multidisciplinary Selection Committee meetings and multidisciplinary discharge planning. Re-assessment: A Transplant Clinical Dietitian is available to re-assess transplant candidates based on the patient's needs and status as identified during the evaluation. Documentation: All assessments will be communicated to the Transplant care team and documented in the recipient's/donor's medical record. Specific responsibilities related to the above will include:
