Admissions Coordinator A healthcare organization is seeking an Admissions Coordinator to support admissions for skilled nursing, homecare, and hospice services. This role serves as a key liaison between referral sources, clinical teams, patients, and families to ensure appropriate placement, timely admissions, and exceptional service. This position is ideal for someone with healthcare experience who excels at coordination, communication, and working in a fast-paced environment. Key Responsibilities * Coordinate admissions for skilled nursing, homecare, and hospice services * Obtain and review pre-admission documentation from referral sources * Review clinical documentation to determine appropriate level of care * Verify physician orders and ensure accuracy of admission records * Communicate with hospitals, physicians, skilled nursing facilities, patients, and families * Develop and maintain relationships with internal teams and community partners * Ensure timely, accurate processing of admissions and referrals * Provide clear, professional customer service both in person and over the phone * Perform additional duties related to admissions and coordination as assigned Qualifications Required * High School Diploma or equivalent * Minimum 1 year of healthcare experience * Knowledge of Skilled Nursing, Homecare, and Hospice admission criteria * Strong organizational, communication, and multitasking skills * Basic computer proficiency * Ability to work independently and as part of a team Preferred * CNA or MA certification * Admissions, intake, or referral coordination experience Job Type & Location This is a Contract to Hire position based out of Woods Cross, UT. Pay and BenefitsThe pay range for this position is $20.00 - $20.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
