| Position Summary |
This position will serve as a Case Manager/Human Services Practitioner with the Encompass First Episode Psychosis Program and
OASIS program. Encompass and
OASIS aim to provide exceptional care and treatment to adolescents and young adults from 15-30 years of age with recent onset psychosis, or individuals at risk for psychosis. Encompass and
OASIS are coordinated specialty care clinics utilizing a team approach to provide wrap-around services to a diverse group of clients from various socioeconomic, cultural, and ethnic backgrounds.
This position will serve individuals and families in Wake, Orange, Durham and surrounding counties. Some interventions may be delivered virtually; however, the majority of the services will be provided in the community, within the identified service areas. This position will focus on the provision of direct case management support for identified Encompass participants, including referrals and linkage to community resources, assistance in navigating care systems, and coordination of services. Case management seeks to improve access to care, reduce barriers and improve outcomes for individuals receiving services. |
| Minimum Education and Experience Requirements |
Associate's degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions. |
| Management Preferences |
- Experience working in the mental health field
- Ability to communicate professionally (verbal and written) to a diverse group of people including team members, stakeholders, and clients.
- Knowledge of local resources, community services and working as a case manager in community settings preferred.
- Experience with data management preferred including input and export of data, data cleaning, analysis, and reporting.
- This position will work at two separate clinics so preference for someone who excels in time management with excellent organizational skills and attention to details.
